Did any violence or potential violence occur?
Was there any conduct in violation of the organization’s policies?
Is there any public policy or legal issues that has been violated?
When was the conduct reported to mgmt?
Does the behavior meet a reasonable person standard of being severe?
How frequent was the behavior?
Was the behavior physically threatening or humiliating or aggressive?
Did the behavior interfere with the complainant’s or other people’s work performance?
What is the position of the accused within the organization?
Did the accused have knowledge of employer’s workplace violence policy?
